Sam Harmel represented the family of a deceased patient at an Inquest into his death.  The patient died on 14th April 2017 when he developed an anaphylactic response to the antibiotic Flucloxacillin given to him in hospital.   

The patient had previously displayed a reaction to the antibiotic hours before he died.  However, due to poor communication and a failure to remove Flucloxacillin from the prescription chart, he was given it again later that evening and suffered a myocardial infarction which led to his death. 

The Coroner concluded that the failures by the Hospital were gross basic failures to provide appropriate medical care and recorded a death of natural causes contributed to by neglect.
